This form can be used to assist instructors in navigating the Student Disability Center’s Fundamental Alteration Process. Please review this process before completing this form. After you have completed this form, a confirmation will be sent to you, as well as the SDC staff member you select on the form. Our staff will review your submission and contact you as soon as possible.
